Forms lets you ask questions and collect the answers - holiday requests, event sign-ups, feedback, job applications and more. Forms are kept in your Files. They come with Premium and Premium+.

Make a form

  1. In Files, click New → Form - or open Forms in the sidebar and click New form.
  2. Pick a starting point - Blank, Contact, Event sign-up, Feedback, Holiday request or Job application - and click Create.

Questions

On the Questions tab, give the form a title and a description, then add questions. Each question can be:

Type For
Short answer A name, a reference
Paragraph Longer answers
Email address Checked to be a real-looking address
Number Numbers only
Date A date picker
Multiple choice (one) One option from a list
Tick boxes (several) Any number of options
Dropdown One option, in a menu

Type the options one per line. Tick Must be answered for questions people can’t skip. Use the arrows to reorder questions, Copy to duplicate one and Delete to remove it. Changes save as you go.

Share it

Open Sharing & settings:

  • Link to fill it in - copy it and send it, or put it on your website.
  • Who can fill it in - only people at your organisation (they sign in, and their name is saved with their answers), or anyone with the link.
  • Ask for their email address - for people who aren’t signed in.
  • Message after sending - what people see once they’ve sent their answers.
  • Taking answers - untick to close the form. The link then says it’s closed.

Open the form at the top shows it exactly as people will see it.

Answers

The Answers tab shows how many answers you have, a chart for each choice question, and every answer in a table with who sent it and when. Click Download CSV to open them in Excel or Google Sheets. Whoever made the form, or an admin, can delete an answer.

Tipp

Your organisation can stop anyone making forms for people outside it - see the Forms only for people in the organisation policy.

Warnung

Never ask for passwords in a form. Smile will never ask you to send one through a form.
